10/25/2023 EnchantaVaughn Approved 1.Behavior: Client greeted QMHP-C as QMHP-C arrived to the home. Client appeared to be in a positive mood while discussing with QMHP-C how his day was going. He verbalized to QMHP-C that his day was going well and that he had a good day in school because he did not get into trouble. Mother verbalized to QMHP-C on how client continues to struggle with managing his anger and his frustrations on a daily basis. 2. Behavior: During the second portion of the session, client needed redirection from QMHP-C as he was leaving the meeting area to go and look out the window. Once he was able to regain focus from redirection he was able to fully participate in the session. 1.Intervention: QMHP-C engaged Client in self-controlling behaviors as well as how to control his behaviors. QMHP-C role-played with Client on his peer interactions and triggers that causes him to act out inappropriately in the school setting. 2. Intervention: QMHP-C modeled effective ways to express feelings and thoughts accordingly and engaging in respectful behaviors. QMHP-C engaged Client in controlling his frustrations and anger control. QMHP-C taught Client different techniques to assist him with showing feelings rather than acting out the behaviors. 1.Response: Client appeared to be receptive to QMHP-Csâ interventions and services that were provided to him during the session. Client struggles with self-controlling behaviors as he acts out impulsively without thinking of the consequences for his actions. 2.Response: Client reported that he has been influenced by his peers as he would act out inappropriately with others. Through role-playing Client was taught to identify his triggers and verbalize his thoughts.
